- comments: no certified food manager was present while tcs foods (hamburgers and chicken) were being prepared and served. Management instructed that a certified food manager must be present at all times while tcs foods are being prepared and served. Priority foundation 7-38-012. Citation issued.

- comments: observed 3 live cockroaches on the floor of the front food prep area underneath the floor mats. Additional pest control service is needed to eliminate the pest activity. Priority foundation 7-38-020(a). Citation issued.

- comments: none of the employees or food handlers were wearing a hair restraint in the food prep area. Management instructed that all food handling employees must wear a hair restraint.

- comments: detail clean the refrigerator and freezer, side of grill and deep fryer, and interior of microwave.

- comments: must provide a drain cover for the floor drain in the front food prep area.
