- comments: observed missing disclosure on food items and missing disclosure statement at bottom of menu for consumer advisory. Instructed to provide an asterik next to the food items that can be served cooked to order and provide a disclosure statement at bottom of menu stating, 'these food items are cooked to order. ' priority foundation. 7-38-005.

- comments: observed ice scoop stored in between ice in ice machine. Must store in a clean ice scoop holder.

- comments: observed dirty cutting boards in kitchen prep area with dark stains and deep grooves. Must clean or replace.

- comments: observed slow drain at hand washing sink at bar area. Must repair.

- comments: previous core violation not corrected from 2/7/2019, report #2261673. #51-unable to identify back flow preventer device on ice machine in rear adjacent to dish washing area. Instructed manager to identify and or install back flow preventer device on ice machine. Maintain at all times. Priority foundation. 7-42-090.
