- comments: observed timed faucet not functioning properly at left sink girls first floor washroom near exit #1, right sink boys first floor washroom near main office. Water must run continuously for at least 15 seconds before reactivating faucet.

- comments: observed 1/4" gap at bottom left corner of right door at exit #2 and bottom left corner of exit door #5. Instructed manager to seal all outer openings on doors to prevent pest entry.

- comments: observed single service utensils not inverted in serving area. Instructed manager to invert all utensils to protect from contamination.

- comments: previous priority foundation violation not corrected from report #2624165 on 9-22-25: 38-observed over 100 mice droppings scattered on floor along walls and corners, behind refrigerator, inside radiator in teacher lounge, under shelving units in chemical storage, on floor in custodial storage in rear lunchroom area, along walls and inside outer floor ledges in lunchroom, along walls under drainbord in kitchen and on floor along walls and corners under shelving in dry storage area.
