2. City of chicago food service sanitation certificate
- comments: observed no certified manager between 1:40 and 1:55 pm while tcs items like sushi were prepared. Instructed to maintain an employee holding a valid city of chicago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012. Citation issued.

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no verifalbe employee health policy on site at the time of inspection. Left template and instructed to maintain copies of health policy signed by all employees on site at all times. Priority foundation violation 7-38-010. Citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no kit and procedure for responding to vomit and diarrhea on site at inspection. Left one pager and instructed to maintain procedure and kit with all necessary supplies, including sanitizer rated as effective against norovirus on site at all times. Priority foundation violation 7-38-005. Citation issued.

36. Thermometers provided & accurate
- comments: observed no thermometer inside front and rear prep coolers. Instructed to provide.

39. Contamination prevented during food preparation, storage & display
- comments: observed cut fish stored in single use grocery store containers. Instructed to use only food grade, reusuable containers.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed both cutting boards in the sushi prep area covered in deep grooves and dark stains. Instructed to repair or replace and maintain smooth, cleanable cutting surfaces.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no chemical sanitizer test strips on site at the time of inspection. Instructed to provide. Priority foundation violation 7-38-005. Citation issued.

50. Hot & cold water available; adequate pressure
- comments: observed no hot water at any sinks throughout the facility. All sinks are maintaining temperatures between 72. 1-78. 9f. Instructed to repair so that all sinks can supply running water under pressure between 100-120f. Priority violation 7-38-030(c). Citation issued.

55. Physical facilities installed, maintained & clean
- comments: observed counters in the prep area covered in cellophane. Instructed to remove.
