Report date: Nov 14, 20258. Hands clean & properly washed
- comments: observed a food handler performing multiple tasks without washing hands. Observed a food handler with gloves on use a fly swatter to kill a roach and then go handle ready to eat food (fries). Also, observed a food handler touching the cash register and putting on new gloves without washing hands. Lastly, the food handler wipes their hands on the apron and nearby wash cloth. Instructed to wash hands after contaminating hands before putting on new gloves and preparing food. Fries were discarded approximately 3lbs at $4. 50. Priority violation 7-38-010, citation issued
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed rear dishwashing sink with no handwashing signage. Instructed to provide and maintain.
38. Insects, rodents, & animals not present
- comments: observed pest activity on site. Found approximately 15 mice droppings, two live roaches, and 4 small flies on site at the time of the inspection. Found mice droppings on the floor surrounding food prep tables, the toilet room, dry storage shelves, and cooking equipment in front and rear prep/dishwashing areas. Found live roaches on the wall behind the food prep table in the rear dishwashing area and behind the two door cooler in the rear. Found small flies on a dish stored above a compartment sink and on boxes stored throughout. Instructed to remove mice droppings, live roach, and small fly activity. Priority foundation violation 7-38-020(a), citation issued

- comments: observed dead roaches on the wall post on the side of the three compartment sink and on the floors. Instructed to remove and maintain.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed missing three sink stoppers for three compartment sink. Instructed to provide and maintain
49. Non-food/food contact surfaces clean
- comments: observed food debris under the flat top grill in the front prep area. Instructed to clean and maintain.
51. Plumbing installed; proper backflow devices
- comments: observed no mop sink on site. Per manager, the handwashing sink in the rear dishwashing area is used as a mop sink. Instructed at least 1 service sink or curbed cleaning facility equipped with floor drain. A floor drain shall be provided and conveniently located for cleaning of mops or similar wet floor cleaning tools and for disposal of mop water or similar liquid waste.
55. Physical facilities installed, maintained & clean
- comments: observed floors throughout in need of cleaning. Instructed to clean and maintain.
