- comments: observed the pest control logbook is not up to date with the required information on site. There was no service receipt that shows the facility has been serviced for this year. Must provide and maintain all required pest control records and documents. Priority foundation violation 7-38. 020 (c), citation issued

- comments: observed the buckets where toys are stored in the infant room with slight debris build up. Instructed to clean and sanitize said equipment more frequently and maintain.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed that facility uses a non-commercial disposal receptacle for the outdoor garbage. Must provide a proper commercial outdoor garbage receptacle from a licensed disposal company. The receptacle must have the facility's name and address conspicously posted. Priority foundation violation 7-38-020(b), no citation issued

- comments: observed a damaged tile near the water fountain in the middle of the children's toilet room. Instructed to repair and maintain.

- comments: observed in the food prep area one of the two light sources had a blown out light bulb and the shield was hanging from the ceiling. Instructed to repair and maintain said light.

- comments: previous core violations not corrected from 03/09/2022, report 2552676 #55 observed openings around pipe fittings under hand washing sinks and floor around floor drain in girls and boys washrooms. Instructed manager to seal all openings and maintain. #55 observed debris on floors along walls and corners in furnace room, utility, electrical and classroom areas. Instructed manager to clean and maintain. #56 observed no mechanical ventilation in boys and girls washrooms. Instructed manager to provide. Priority foundation:7-42-090, citation issued
