- comments: 2-501. 11 observed no clean up supplies on the premises to respond to vomit and diarrheal events. Management instructed to provide all listed items on the clean up procedure. Priority foundation 7-38-005.

- comments: 5-204. 11 observed no exposed hand washing sink provided behind the bar. Management instructed to install an exposed hand washing sink in this area. Priority foundation 7-38-030(c).

- comments: 7-206. 12 observed rodent bait pellets scattered on the floor throughout the basement. Management instructed that all rodent poison must be stored in covered, tamper-resistant bait stations. Priority 7-38-015. Citation issued.

- comments: 6-202. 15 observed a 1/2' gap along the bottom of the rear and front exit door and the basement door. Management instructed to rodent proof all outer openings.

- comments: 5-501. 17 observed no covered waste receptacle in the women's washroom. Management instructed to install.

- comments: 6-501. 114 observed excessive clutter in the storage room of the basement so that the whole basement could not be inspected due to several piles of wood pallets being stored on the floor. Management instructed to remove any clutter that is contributing to the rodent harborage conditions in the basement.

- comments: 6-201. 11 observed broken floor tiles in the rear storage room. Management instructed to replace the broken floor tiles. Observed raw wood as a floor covering behind the bar. Management instructed to install a smooth and easily cleanable floor covering behind the bar.

- comments: observed a food handling employee without a food handler certificate. Management instructed that all food handling employees must show proof of training.
59. Previous priority foundation violation corrected
- comments: 8-404. 13(b)(3) previous priority foundation violation from report #2497391 on 3/25/21 not corrected: 38 - observed over 200 rodent droppings on the floor throughout the basement storage rooms and boiler room. Also observed approximately 10 rat droppings under the unused stove in the kitchen area, 30 rat droppings in the corner of the kitchen area, and 30 rat droppings on the floor behind the bar. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority 7-42-090. Citation issued.
