- comments: observed no hand washing signs at hand washing sinks in prep, dishwashing and washroom areas. Instructed manager to provide hand washing signs at all hand washing sinks used by employees.

- comments: observed single service articles stored on floor in prep area. Instructed manager to elevate all items six inches off floor.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed cardboard used as liner on shelving units n prep area and floor in dish washing. Instructed manager to remove. All food and non-food contact surfaces shall be smooth and easily cleanable.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered waste receptacle in washroom. Instructed manager to provide.
55. Physical facilities installed, maintained & clean
- comments: observed missing and damaged baseboards near hand washing sink and prep station in front prep area. Observed missing floor drain cover in rear storage area. Instructed manager to replace and maintain. Observed openings around pipe fittings under hand washing sinks. Must seal all openings and maintain.
