- comments: observed no hand washing signage at hand washing sink in prep area and in bathroom. Must provide.

- comments: noted paper towel at the hand wash sink not properly dispensing the paper towel thus employees unable to dry their hands after washing. Instructed to have a working hand wash sink dispenser, paper towel or drying equipment for employees to dry their hands at all times. Priority foundation violation #7-38-030(c).
38. Insects, rodents, & animals not present
- comments: noted over 20 mice dropping on floor along wall base inside the employee bathroom, on shelf underneath the prep table and on floor at same. Instructed to detail clean and sanitize affected areas, eliminate pest activity. Instructed to consult with pest control company and have premises treated. Priority foundation #7-38-020(a)

- comments: noted wash rooms on premise with garbage receptacles and no lids. Instructed to provide trash cans with lids at all women wash rooms and unisex wash rooms

- comments: instructed to clean and maintain floors along wall base and underneath equipment especially at areas where mice droppings were found.
