- comments: noted no metal stem thermometer on site to monitor the internal food temperature of tcs food. Facility must obtain and maintain proper thermometers. Priority foundation violation #7-38-005. Citation issued.

- comments: observed pest control service receipt from company without proper idph license number. Logbook must include all content prescribed in the rules of the department of health. They are including but not limited to copy of pest control illinois department of health license number, written verification of certified pest technician responsible for using pesticides, a record of all visits to the retail food establishment. Facility must provide logbook and maintain. Priority foundation violation. 7-38-020 (c). Citation issued.

- comments: observed toilet room door to not be self closing. Instructed to install a self closing device and maintain at all times.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed no commercial dumpster contract. Instructed to provide a contract with a licensed waste management operator. Priority foundation violation #7-38-020(b). Citation issued.

- comments: observed broken and stained ceiling tile panels in the back. Instructed to repair, or replace broken panels and replace the stained ceiling tiles.
