- comments: observed live pest activity in the food prep area and 2nd floor dining room. Observed 12 live small flying insects on the walls and ceiling in the food prep area and mop closet next to the food prep area. Also observed 2 live cockroaches on the floor of the 2nd floor food service station in the dining room. Additional pest control service is needed to eliminate the pest activity in all areas. Priority foundation violation 7-38-020(a). Citation issued.

- comments: clean and delime the interior of the high temperature dish machine.
51. Plumbing installed; proper backflow devices
- comments: repair the cold water handles for the exposed hand washing sinks in the food prep and dish washing area.
56. Adequate ventilation & lighting; designated areas used
- comments: observed inadequate lighting in the dish washing area and several lights that are out in the food prep area and outside of the walk-in cooler. Management instructed to provide adequate lighting in all food prep and ware washing areas.
