- comments: 6-301. 12 observed no hand drying provision located at both hand washing sinks in the food prep area and the hand washing sink in the toilet room. Instructed to provide at all times. Priority foundation. Citation issued under violation above.

- comments: 6-301. 11 observed no hand washing cleanser at the front food prep area hand washing sinks. Instructed to provide at all times. Priority foundation. Citation issued under violation above.

- comments: 5-205. 11 observed a hand washing sink blocked with a bucket and cutting board being stored inside of the hand washing sink. Instructed to never block hand washing sinks, all hand washing sinks must be accessible at all times. Priority foundation. Citation issued 7-38-030(c).
16. Food-contact surfaces: cleaned & sanitized
- comments: 4-303. 11(a) observed facility has no adequate sanitizer on site and was washing dishes at the 3 compartment sink with only soap and hot water. Instructed facility must maintain adequate sanitizer on site at all times the facility is open and operating. Manager went to the store and bought bleach chlorine during the inspection. Priority violation. Citation issued 7-38-005.
22. Proper cold holding temperatures
- comments: 3-501. 16(a:2,&b) observed tcs foods at inadequate cold holding temperatures located in the food prep area; observed about 10 lbs of cooked rice at 60f, observed about 15 lbs of raw chicken at 62f, observed about 3 lbs of cheese at 61f and observed about 10 lbs of milk at 59f. Instructed to discard and denature said tcs foods and always maintain all cold holding tcs foods at 41f or below at all times. Manager discarded said tcs foods during the inspection. Priority violation. Citation issued 7-38-005.
38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of pest activity on site. Observed about 25 rodent droppings in the furnace room. Instructed to clean and sanitize affected area and recommended contact pest control for a service. Priority foundation. Citation issued 7-38-020(a).
40. Personal cleanliness
- comments: 2-402. 11 observed food handlers not wearing hair restraints at the beginning of the inspection. Manager provided hair nets during the inspection.
41. Wiping cloths: properly used & stored
- comments: 3-304. 14 observed wiping cloths being stored on a cutting board attached to a prep cooler. Instructed to store all wiping cloths in a sanitizing solution at all times.

- comments: 5-501. 116 observed the exterior of the garbage cans with dirt debris located in the food prep area. Instructed to clean.
