- comments: observed food debris on floors throughout the walk-in cooler and rear prep area. Instructed the person in charge to detail clean floors and maintain.

- comments: observed two certified food service managers without allergen awareness training certificates. Instructed the person in charge to ensure that any individual who has a city of chicago foodservice managers certificate obtains allergen awareness training and certificate.
60. Previous core violation corrected
- comments: previous core violations from 05/08/2023 inspection, report #2575516 not corrected: #39-observed food containers stored on the floor in the walk-in cooler, prep and storage areas. Instructed the person in charge to properly store and protect food items from contamination while stored by elevating at least six inches from the floor on raised shelving units. #47-observed multiple raw wood table tops in the rear prep area. Instructed the person in charge to seal or paint to provide a smooth and easily cleanable surface and maintain. #47-observed rusted storage racks in the walk-in cooler in the rear prep area. Instructed the person in charge that all nonfood-contact surfaces must be smooth and easily cleanable. #57-observed multiple food handling employees with expired food handler certificates. Instructed the person in charge to renew expired food handler certificates to comply with idph food handler requirements. 7-42-090 priority foundation violation. Citation issued.
