- comments: observed ice machine interior surface soiled with black substance. Instructed to dispose of ice, clean and sanitize. Priority foundation violation. 7-38-005. Citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed food items stored in containers in walk-in freezer and prep area coolers uncovered. Instructed to provide food grade lids and/or covers. Observed food containers stored on the floor in the walk-in cooler, walk in freezer and storage areas. Instructed to store properly store by elevating at least six inches from the floor on raised shelving units.

43. In-use utensils: properly stored
- comments: observed scoops for dry storage containers stored directly inside of the container. Instructed to store properly between usage.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed aluminum foil used on food items stored in the walk-in cooler and freezer and prep coolers. Instructed to use food grade storage bags/plastic wrap or food grade lids.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle in employee washroom. Instructed to provide cover.

55. Physical facilities installed, maintained & clean
- comments: observed mops drying/stored improperly. Instructed to store properly in mounted position to dry properly between usage and prevent pest harborage.
