- comments: observed no valid original city of chicago foodservice sanitation certificate on site at time of inspection. Instructed person in charge that valid, original certificate as well as certified manager must be on site at all times. Priority foundation 7-38-012. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed rear kitchen handsink to be inaccessible with the basin filled with food. Instructed person in charge to maintain all handsinks accessible at all times. Priority foundation violation #7-38-030(c). Citation issued.

- comments: observed build up of mold like substance in ice machine located in basement. Instructed person in charge to clean and maintain. Priority foundation #7-38-005. Citation issued.
