- comments: observed two live cockroaches on table in dining area. Instructed to remove roaches, clean surfaces, and maintain premises free of pest. Recommended receive additional pest control service. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed rust on shelving unit in the walk-in cooler and in the walk-in freezer located in the rear, instructed the person in charge to remove rust and maintain in good repair.

- comments: observed accumulated grease and food debris on interior and exterior surfaces of cooking equipment, prep tables, coolers, freezers, shelving units. Instructed the person in charge to clean and maintain. (nonfood contact surfaces)

- comments: observed food debris/spills on floor throughout preparation area. Instructed the person in charge to detail clean and maintain.

- comments: observed two certified food managers with expired allergen awareness training. Instructed the person in charge to renew expired allergen awareness training and ensure that any individual that holds a city of chicago food sanitation certificate receives allergen awareness training and receives allergen awareness certificate.

- comments: previous core violations from 05/26/2022 inspection, report #2556338 not corrected: #39-observed no splash-guard between handsink that is located directly next to the grill. Instructed to provide splash-guard to prevent contamination of food from grill and maintain. #47-observed cutting boards in kitchen preparation area with deep scratch marks. Instructed to resurface or replace and maintain. 7-42-090 priority foundation. Citation issued.
