- comments: observed no pest control log book on site. Logbook must consist of a record of visits and a copy of the idph license number. Must provide and maintain all required pest control records and documents. Priority foundation violation 7-38-020 (c), citation issued

39. Contamination prevented during food preparation, storage & display
- comments: observed the deep fryer with grease not covered while not in use at the time of the inspection. Instructed to cover and maintain, in order to prevent potential contamination.

55. Physical facilities installed, maintained & clean
- comments: observed slight grease build up on the floor under the deep fryer. Also, observed slight debris on the floor under the drainboard of the high temperature dishwashing machine. Instructed to clean floors and maintain.
