- comments: observed bulk containers not labeled. Instructed manager to label all bulk containers with common name of food.

- comments: observed over 40 mice droppings scattered on floor along walls and corners behind chest freezer, over 40 mice droppings scattered on bottom surfaces of desk, shelving units, on floor along walls and corners in office. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed 1/4' gap on bottom of overhead door in rear prep area. Instructed manager to seal all outer openings on door to prevent pest entry.

- comments: observed food debris and grease buildup on interior and exterior surfaces of coolers, freezers, holding units, bulk containers, shelving units, cooking equipment. Instructed manager to clean and maintain.

- comments: observed missing ceiling tiles in rear prep and office area, and openings along baseboards near serving area entrance, openings around pipe fittings under rear hand washing sink. Instructed manager to seal all openings in walls and ceilings and maintain.
