- comments: found no label on the large bulk containers in the pizza prep area; must label containers and maintain.

49. Non-food/food contact surfaces clean
- comments: observed grease and dust at the bottom of the fryers; instructed to clean. Observed the side exterior areas of the fryers in need of cleaning; instructed to clean. Observed dust on the fan cover of the walk-in cooler; instructed to clean to remove dust.

51. Plumbing installed; proper backflow devices
- comments: observed a leak from the faucet neck of the exposed handsink in the rear prep area; instructed to repair leak.

55. Physical facilities installed, maintained & clean
- comments: observed the wall coving at the base of the wall behind the prep table in the front prep kitchen loose; instructed to reattach coving to wall.

- comments: observed expired food handler certificates; instructed to have all food handlers obtain valid certificates.
