- comments: no city of chicago certified food manager on site during the inspection while tcs ice cream was being served. Such a person must be on site when foods are prepared and served. Priority foundation violation 7-38-012 citation issued.

- comments: noted inadequate hot water in the west washroom. Maximum hot water temperature noted at 69. 4f. Must provide hot water between the temperature of 100f to 120f. Priority violation 7-38-030(c) citation issued.

- comments: must not use duct tape as a means of repair on floor at the threshold to the front food prep area. ----floor under the rear one compartment sink with dirt and debris. Must clean and maintain. -----missing ceiling tiles above the rear walk-in freezer. ----missing floor tiles at the rear utility service sink. Must replace all.
