- comments: observed no person in charge with valid chicago food sanitation certficate. Instructed manager to desginate person in charge during all hours of operation. Priority foundation

- comments: observed no chicago food sanitation manager on site or valid certificate posted when tcs food(pasta,yams) prepared and served. A chicago food sanitation manager must be on site and valid certficate posted at all times tcs food prepared and served. Priority foundation 7-38-012 citation issued
6. Proper eating, tasting, drinking, or tobacco use
- comments: observed food handler eating taco in food prep area. Instructed manager to provide separate area outside of prep area for employee meals.

- comments: observed bulk containers not labeled in prep area. Instructed manager to label all bulk containers with common name of food.
39. Contamination prevented during food preparation, storage & display
- comments: observed food items stored on floor in prep area. Instructed manager to elevate all food items six inches off floor.

- comments: observed torn syrofoam cup used as dispensing utensil for jerk sauce. Instructed manager to provide proper long handle dispensing utensils to prevent contamination.

- comments: observed single service articles stored on floor in prep area. Instructed manager to elevate all single service articles six inches off floor.

- comments: observed rust deposits on bottom of prep tables. Instructed manager to repair and maintain.

- comments: observed food debris on interior and exterior surfaces of cooking equipment, prep tables, coolers, freezers, shelving units. Instructed manager to clean and maintain.

- comments: observed leaking drainpipe under 3-compartment sink. Instructed manager to repair and maintain.

- comments: observed debris on floors under all equipment, along walls and in all corners in prep, storage and washroom areas. Instructed manager to clean and maintain.

- comments: observed no food handler training for employees on duty. Instructed manager to provide food handler training for all employees.

- comments: previous core violations not corrected from report #2580196 on 8-16-23: 55-must provide a hooking system for wet mops. 56-must place cooking equipment under ventilation hood. Priority foudnation 7-42-090 citation issued
