- comments: observed dead roaches on site. Dead roaches observed on floor near rear exit door, on the cart with clean pans, on the lower wall under the drainboard of the three compartment sink. Instructed to remove dead roaches, clean and maintain.

- comments: observed the shelves under the cooking equipment and hot holding equipment in need of cleaning. Instructed to clean and maintain.

- comments: observed no allergen on site for the certified food manager. Instructed all food managers must take course and maintain records on site.
60. Previous core violation corrected
- comments: observed previous core violations not corrected from 05/09/2025 , report 2684305 49: observed the handles, door and the inside of coolers dirty. Must clean and maintain. 51: observed the rear hand sink with a leaky pipe underneath. Must repair and maintain. 55: floors, walls and ceiling tiles need cleaning through out to remove spills, debris and/or grease. 56: must clean the hood and filters above the cooking equipment to remove debris.
