- comments: observed no clean-up supplies on the premises to respond to a vomit or diarrheal event. Management instructed to provide. Priority foundation violation 7-38-005. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the exposed hand washing sink next to the 1 compartment prep sink not accessible for use. Management instructed to repair the exposed hand washing sink. Priority foundation violation 7-38-030(c). Citation issued.

- comments: observed no hand soap available at the exposed hand washing sink in the food prep area. Management instructed to provide hand soap at all hand washing sinks at all times. Priority foundation violation 7-38-030(c). See violation #10 above for citation.
38. Insects, rodents, & animals not present
- comments: observed live insect activity in the food prep area. Observed 3 live cockroaches on the wall, ceiling, and door of the refrigerator. Also observed 2 live small flying insects on the walls of the food prep area. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a). Citation issued.
41. Wiping cloths: properly used & stored
- comments: observed wet wiping cloths stored on top of the prep tables and prep cooler cutting boards. Management instructed to store all wet wiping cloths in a container of sanitizer when not in use.
49. Non-food/food contact surfaces clean
- comments: clean the interior of the 3 door prep cooler.
55. Physical facilities installed, maintained & clean
- comments: clean the grease on the floor under the deep fryers.
56. Adequate ventilation & lighting; designated areas used
- comments: clean the grease buildup on the ventilation hood and filters above the cooking equipment.
58. Allergen training as required
- comments: observed the certified food manager without an allergen training certificate. Management instructed that all certified food managers must show proof of allergen training.
