- comments: raw concrete floor noted through out rear. Instd to repair floor and wallbase. Instd to seal raw concrete floor so as surface to be smooth, even, and easily cleanable. Provide outlet covers for all outlets and maintain. Door frame leading to prep area with exposed wood and wall studs. Wall coving is loose and missing in various areas of the prep area, exposing gaps between the floor and the wall. Instd to provide wall base and maintain
59. Previous priority foundation violation corrected
- comments: priority foundation from 10/18/24 report 2605326 not corrected. Over 100 live flies were noted in display cases and on walls and ceiling in front of the customer service/display area and the prep area. Over 20 live roaches were noted in the following areas: large roaches crawling on the wall next to the prep table and on the floor in the prep area. Baby roaches were noted crawling on the dough mixer and the wall next to the prep table. Large roach noted on shelf behind microwave. Numerous dead roaches were noted in the condensor tray of the cake display cooler and on dry storage containers and storage shelves. Instd to remove all flies and roaches. Clean and sanitize all affected areas and contact pest control for updated pest control receipt. Citation issued priority 7-42-090
