- comments: plastic ice shoot on the dining room ice dispenser noted dirty, in need of cleaning. Must clean, sanitize and maintain.
41. Wiping cloths: properly used & stored
- comments: noted stored wiping cloth hanging on inside and out of closed door on the front prep three door cooler. Must properly store dry wiping cloths on a clean surface.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must not use milk crates as a means of elevation for stored chemicals under the three compartment sink. Floors must be accessible for cleaning and monitoring.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: no quaternary chemical test strips on site for the quaternary sanitizer provided on site. Must provide quaternary test strips for quaternary sanitizer. Priority foundation violation 7-38-005 citation issued.

- comments: interior bottom of the rear two door tall reach-in cooler with excessive pooling liquid and food debris. Must clean, maintain and remove pooling liquid.

Must replace/repair. 6-201. 11--noted dirty ceiling light shield in the rear storage room. Must clean and maintain. #57-no proof of food handler training or certificate for the employees on site. Must provide for all food handlers. Priority foundation violation 7-42-090 citation issued.
