- comments: observed no verifiable employee health policy on premises. Instructed to provide proof of verifiable employee health policy. Priority foundation violation 7-38-010 citation issued.
37. Food properly labeled; original container
- comments: observed salt stored in orange bucket not labeled with common name. Instructed any food not in original container shall be labeled with common name of food.
38. Insects, rodents, & animals not present
- comments: observed no complete pest control log book on premises. Observed most recent pest control inspection reports dated 5/21/2020 and 8/13/2020 and no current idph license for pest control company in pest control log book. Instructed to provide complete pest control log book and maintain on premises at all times. Priority foundation violation 7-38-020(c) citation issued.

- comments: observed openings at base of rear exit door and front entrance door on northside of facility. Instructed to seal all outer openings to prevent pest entrance.

- comments: observed unsealed wooden planks in potato prep area. Instructed to seal wooden planks for ease of cleanability. Observed pallets used for potato storage. Observed chemicals stored on floor underneath three compartment sink. Instructed to obtain adequate shelving to elevate all food and non food items at least six inches off of floor for ease of cleanability.

- comments: observed orange home depot bucket used to store salt in front prep area. Instructed to provide food grade container for salt storage.

- comments: observed onion cutter in rear prep with food debris on blade. Instructed to clean and maintain.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed clutter in refuse area, unused items include nonworking fryers, blue containers, red barrels and pallets. Instructed to remove unneccessary items, clean and maintain refuse enclosure.
55. Physical facilities installed, maintained & clean
- comments: observed stained, missing or damaged ceiling tiles throughout facility. Instructed to replace ceiling tiles and maintain. Observed damaged floor in dishwashing area and in walkin cooler. Instructed to smoothly repair damaged floor in dishwashing area and in walkin cooler for ease of cleanability.

- comments: observed vent cover above exit door with heavy dust accumulation. Instructed to detail clean and maintain.
