- comments: pic doesn't have a certified food managers certificate must provide and maintain. (priority foundation 7-38-012)

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs foods are being prepared,handled and served such as (chicken,coleslaw,french fries, fish,etc)must be on site at all times. (priority foundation 7-38-012)(citation issued)

36. Thermometers provided & accurate
- comments: must provide internal thermometers for prep coolers.

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 50 or more live small flies inside of rear mopsink and walls and flying around,on boxes in front prep area,men's and women's toilet rooms. Recommended to have a pest control operator to service premises on or after 11/4/24. (priority foundation 7-38-020(a)(citation issued)

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no chemical test kit to check sanitizing solution ppm's. Must provide and maintain. (priority foundation 7-38-005)(citation issued)

- comments: must clean debris build up from garbage corral in dining area & reach in freezer.

- comments: must repair or replace missing draine line pipe at 1- compartment sink.

- comments: observed toilet in men's toilet room not flushing and one in women's toilet room when flushed water comes shooting out front the lid of toilet bowl and pooling on to floor must repair and maintain. (priority foundation 7-38-030(c)(citation issued)

- comments: must repair or replace missing ceiling tiles throughout premises,loose wall panel outside of toilet rooms.

Must grout floor around cooking equipment,mop sink basin, at 3- compartment sink. #55-6-501. 16 : must provide a hooking system to hang up wet mops. #55-6-501. 114 : must elevate articles off of floor in unused walk in coolers. 56- 2-102. 13 : observed no food handlers training for employees must provide and maintain. (priority foundation 7-42-090)(citation issued)
