- comments: observed no certified food manager on duty while tcs foods are being prepared,handled and served such as (chicken,coleslaw,french fries, fish,etc)must be on site at all times. (priority foundation 7-38-012)(citation issued)

- comments: must provide internal thermometers for prep coolers.

- comments: observed evidence of appx. 50 or more live small flies inside of rear mopsink and walls and flying around,on boxes in front prep area,men's and women's toilet rooms.
