2. City of chicago food service sanitation certificate
- comments: no city of chicago food service sanitation certificate was posted nor provided on site for the present manager on duty while tcs foods such as deli sandwiches prepared and served on site. No valid doccumentation on premises. Instructed to provide. A least one certified food manager must be on site when food is handled. Priority foundation violation 7-38-012 citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: a sign or poster that notifies food employees to wash their hands is not provided at handwashing sink in dish room used by food employees. Provide a clearly visible handwashing sign at each hand sink.

39. Contamination prevented during food preparation, storage & display
- comments: food storage containers stored on the floor in kitchen, hallway near dish, and outside storage area. Items must be placed 6 inches above the floor.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: quat. Test strips is not available to the employees to measure sanitizing solution concentration for 3compartment sink sanitizer. Facility must provide a test kit or other measuring devices for the employees can measure the sanitizer concentration and avoid a concentration that is too high, which can be toxic. Priority foundation violation 7-38-005 citation issued.

- comments: kitchen equipment (handles on prep units) is soiled with food debris. Facility must clean and maintain.

- comments: no hot water on premises. Water temperature measured below 70f. Priority violation 7-38-030 (c) citation issued.

- comments: observed outside receptacles heavily soiled with garbage accumilation around dumpsters. Facility must clean and maintain area. Priority foundation violation 7-38-020 (b) citation issued.

55. Physical facilities installed, maintained & clean
- comments: standing water observed on the floor in the main prep area. Facility must remove standing water, clean, and maintain.

60. Previous core violation corrected
- comments: previous core violations have not been corrected dated on 11/7/22. Report #2566643. #52) observed no grease trap connected to 3 compartment sink. Instructed to install in accordance with all relevant laws, rules, and regulations. #57) observed no food handler training certificates on site at time of inspection. Instructed all food employees must complete food handler training and maintain records thereof on site at all times. #58) observed no allergen training on site at time of inspection. Instructed all employees holding a city of chicago foodservice manager certificate must complete allergen training and maintain records thereof on site at all times. Repeat core violation. Observed the following repeat core violation not corrected from report #2472357 dated 1/6/2021 as well. Priority foundation violation 7-42-090. Citation issued.
