- comments: no hot running water in the men's bathroom. Instructed to provide and maintain att at all times. Priority violation 7-38-030(c)

21. Proper hot holding temperatures
- comments: observed tcs foods: white rice at improper temperatures, stored on the prep counter and in a hot box. Temperatures ranged from 102. 0 f to 112. 5 0f. Food valued at $20. 00. Manager voluntarily discard & denatured food. Instructed to hold hot tcs foods at 135f or above. Priority violation 7-38-005 citation issued.

22. Proper cold holding temperatures
- comments: noted internal temperatures of the following time/temperature control for safety food items inside the food display cooler to be improper: cut up tomatoes 66. 51 f, cooked corn 103. 0 f, cooked potatoes 85. 00f, cooked quinoa 68. 0 f. Manager immediately and voluntarily disposed of 15 lbs of products worth $60. 00 through denaturing process. Priority violation #7-38-005. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed approximately 25-30 live small flying insects at the site during inspection in the dishwashing area under 3 3-compartment sink. Recommend contacting pest control. Priority foundation violation. 7-38-020(a). Citation issued.

41. Wiping cloths: properly used & stored
- comments: noted dirty wiping cloths on prep tables at front and rear prep areas not properly stored. Instructed to properly store all washcloths in a sanitizing solution, especially when not in use.

55. Physical facilities installed, maintained & clean
- comments: observed food build-up on cooking equipment. Instructed to detail clean and maintain all cooking equipment and maintain at all times.

58. Allergen training as required
- comments: noted certified food managers on duty with no allergen training certification. Instructed management that all food service managers are required to have the allergen training certification.

61. Summary report displayed and visible to the public
- comments: 'observed no inspection report summary posted on the premises. Management was given a copy of today's inspection report and was instructed to post it in public view. Priority foundation. : 7-42-010(b), citation issued.
