- comments: pic doesn't have a certified food managers certificate must provide and maintain. (priority foundation 7-38-012)

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs foods are being prepared,handled and served such as (pizza,egg whites,milk,etc)must be on site at all times. (priority foundation 7-38-012)

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed hot running water temperature at hand sinks in 1st fl. Girl's handicapp stall at 75. 2f. 1st fl. Boy's 51. 1f,75. 6f & 84. 2f must have hot running water at 100. 0f must repair and maintain. (priority 7-38-030)

10. Adequate handwashing sinks properly supplied and accessible
- comments: must adjust faucet handles to stay on longer in 3rd fl. Staff toilet room.

38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 100 or more mice droppings in kitchen-on conduits and wall outlet covers near ovens,on floor behind oven,in corner near storage rack. Office-on conduit lines on wall,on floor along wall bases of file cabinets,locker and radiator. Dry food /chemical storage-along wall bases and under shelving units. Mopsink closet-in between clutter and behind door. Lunchroom-on floor under radiator next to milk cooler. Pre-k 103-on floor & on top of shelving unit in interest area. Must remove droppings clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 1/27/25. (priority foundation 7-38-020(a)

- comments: observed no pest control log book with receipt of service and required docments must provide and maintain. (priority foundation 7-38-020(c)

- comments: must elevate food from floor in vegetable walk in cooler.

- comments: must clean dust build up from pre-k 103 shelving units and cubbies.

- comments: must provide garbage cans with lids in 3rd fl. Girl's.
