- comments: 6-301. 14 observed no handwashing sign at the handwashing sinks in the food prep area and in the toilet room. Instructed to provide.

- comments: 3-601. 11 observed prepackaged foods to go without labels containing the business name business address and ingredients. Instructed to provide.
38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed evidence of pest activity on site. Observed about 30 rodent droppings in the corner by the dry storage shelves in the rear. Instructed to eliminate pest activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 4-301. 13 observed no adequate drainboard at the 3 compartment sink rear area. Instructed to provide.

- comments: 4-601. 11 observed all refrigeration units in need of cleaning to remove debris. Instructed to clean.

- comments: 5-501. 15 observed the outside garbage dumpster overflowing with garbage and lids not tight fitting. Instructed never allow outside garbage dumpster to overflow and always maintain lids tight fitting. Priority foundation. Citation issued 7-38-020(b).

- comments: 6-501. 12 observed floors throughout with accumulated dirt debris. Instructed to clean floors in the rear by the 3 compartment sink and in the customer service area.
