- comments: must provide hand washing signs at hand sinks in 1st fl. Men's and women's toilet rooms.
38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 100 or more mice droppings on ledges around motor of cold holding unit, shelving unit next to dishes on cold holding line, on floor behind grease trap. Must remove droppings, clean, and sanitize areas. Recommended to have a pest control operator to service premises on or after 10/15/25(priority foundation 7-38-020(a)(citation issued)

- comments: observed a gap along bottom of doors #4 and in corners of #2. Must make doors tight fitting to prevent insect entry.
51. Plumbing installed; proper backflow devices
- comments: must provide a backflow device on ice machine to be seen for servicing.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: must provide a self closing device on toilet room door that leads into prep area.
