16. Food-contact surfaces: cleaned & sanitized
- comments: observed the ice bin interior in need of cleaning. Instructed to remove ice, clean and maintain at all times.

38. Insects, rodents, & animals not present
- comments: observed pest control log book with reports from 01/2024, 05/2024, and 11/2024 service reports. All retail food establishments must maintain a log book which shall be kept for a period of twelve (12) months and include a copy of the service tickets and the pest control company�s illinois department of public (idph) health license number. Instructed to provide. Priority foundation violation #7-38-020(c).

39. Contamination prevented during food preparation, storage & display
- comments: observed food containers stored on the floor in the walk-in cooler and prep area. Instructed manager to properly store and protect food items from contamination while stored by elevating at least six inches from the floor on raised shelving units.

43. In-use utensils: properly stored
- comments: observed ice scooper being stored in ice bin with handle touching ice in ice bin. Instructed on how to properly store ice scoop between uses.

49. Non-food/food contact surfaces clean
- comments: observed an excessive amount of debris and ice buildup on the chest freezer in storage area. Must remove excess ice, detail clean, and maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: observed a burned out light-bulb in hood system. Instructed to replace bulb and maintain lighting in the unit.
