- comments: 4-601. 11(a) observed a meat slicer with food and debris built on food contact surfaces located in the basement. Instructed to wash, rinse and sanitize after every use. Priority foundation violation #7-38-005 consolidated with the above violation.

- comments: 5-205. 15 observed drainpipe leaking under the hand washing sink in the front kitchen area. Instructed to repair leaking drainpipe.

- comments: 6-201. 11 observed missing ceilings tile panels in the rear storage/employee room in the basement and water-stained ceiling tiles in the employee toilet room. Instructed to repair and replace missing panels and-or replace the stained ceiling tiles.
