5. Procedures for responding to vomiting and diarrheal events
- comments: observed facility with a blood borne fluid spill kit and no corresponding clean up procedure for responding to vomiting and diarrheal events. Instructed facility to develop a clean-up procedure and maintain appropriate supplies that correspond with on-site plan. Must keep both the policy and supplies on site at all times. Priority foundation violation #7-38-005. Citation issued.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed ice machine located in prep area with a build-up of black slime substance, dripping onto ice cubes stored at below ice bin. Ice is used for human consumption. Also black slime at exterior of unit between ice bin and top part of the unit. Instructed to clean, sanitize and maintain. Priority foundation violation #7-38-005. Citation issued

38. Insects, rodents, & animals not present
- comments: observed pest activity with approx. 25 small flies noted during inspection. Small fly activity was noted in the prep, customer waiting, and upstairs customer seating area. Instructed to consult pest control, as additional services are needed to minimize or eliminate activity before re-inspection. Priority foundation violation #7-38-020(a). Citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed no splashguard between prep area and exposed hand sink and 3 compartment sink. Must install a washable splashguard.

49. Non-food/food contact surfaces clean
- comments: observed the interior of the reach in coolers with excessive food debris. Instructed to detail clean.

51. Plumbing installed; proper backflow devices
- comments: observed no utility sink in facility pic indicated staff are instructed to throw wastewater from mop buckets in alley, noted pic also indicated they fill mop bucket up by pitchers at 3 comp sink. Pic indicated next door dentist allows them to use utility sink in basement but they don't use it and basement room as a dry storage. Instructed to provide for proper disposal of dirty mop water.

55. Physical facilities installed, maintained & clean
- comments: observed clutter stored on floor in toilet room which was made into storage area. Noted single use items (paper towels, oatmilk, and single service items) instructed manager to remove tthese items may not be stored in toilet room.

57. All food employees have food handler training
- comments: observed no proof of training for all food handlers. Instructed to provide and maintain records at all times.

60. Previous core violation corrected
- comments: noted continued non-compliance from report #2575298 conducted on may 3, 2023. Previous core violations not corrected: (#47) large chest freezer door rubber gasket ripped. Must repair/replace door rubber gasket. Must correct core violations by the date on the report. Priority foundation violation #7-42-090. Citation issued.
