- comments: 6-202. 15 observed the front and rear exit doors in poor repair. Must provide and maintain a weather strip to prevent possible entry point for pests

- comments: 4-602. 13 must detail clean and maintain the interior of the coolers, freezers and storage shelving.
55. Physical facilities installed, maintained & clean
- comments: 6-501. 12 must detail clean the walls, floors and ceiling to remove food splatter, spills, debris and/or grease. Also clean the air vents, fan blades through out to remove debris and grease.
56. Adequate ventilation & lighting; designated areas used
- comments: 4-301. 14 must clean the hood above the pit to remove debris

- comments: 2-102. 13 instructed manager to comply with idph food handler requirment or citation will follow.

It is as follows. . . Observed no proof of pest control service for facility. Instructed to provide agreement and service reports updated in logbook as required by code. Priority foundation violation. Instructed to correct. #7-42-090(c). Citation issued noted on 7/17/25 observed evidence of pest activity on premise (approximately 100). License suspended. Office notified. When all violations are 100% in compliance, please contact cdph. Reopening procedures discussed, documents provided. Priority violation. #7-42-090. Citation issued.
