Flip top; cooked chicken (45F - Cold Holding); cooked pork (45F - Cold Holding); cooked beef (53F - Cold Holding). Per operator foods not portioned or prepared today, in unit less than 3 hours. Operator moved to reach in cooler for quick chill. Observed kitchen flip top not working as intended **Corrected On-Site**

- High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. **Repeat Violation** **Admin Complaint**
