- comments: observed no person in charge at time of inspection with foodservice manager training.
2. City of chicago food service sanitation certificate
- comments: observed no valid city of chicago foodservice sanitation certificate on site at time of inspection. Instructed person in charge that certified maanger as well as certificate is required on site at any time tcs foods are being prepared or served(eggrolls, chicken tacos). Priority foundation 7-38-012. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: found there to be no paper towels/hand drying provision at handsinks throughout facility and restrooms. Instructed person in charge that hand drying provision is to be provided and maintained at all times. Pic obtained paper towel for hand sink during time of inspection. Priority foundation violation #7-38-030(c). Citation issued.
38. Insects, rodents, & animals not present
- comments: observed no current pest control log book on the premises at time of inspection. Instructed person in charge that a licensed pest control company and operator must provide at least 2 pest inspections a year and the reports must be maintained in the log book. Instructed to provide a current pest control log book and pest inspection reports. Priority foundation 7-38-020(c). Citation issued.
49. Non-food/food contact surfaces clean
- comments: observed bottoms of reach in coolers in front area of store to have rust. Instructed person in charge to repair to be smooth and easily cleanable.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed outside dumpster full with trash overflowing and piled up alongside and along alley behind facility. Instructed perosn in charge to remove all trash and maintain area clean and clear. Priority foundation violation #7-38-020(b). Citation issued.
55. Physical facilities installed, maintained & clean
- comments: observed ceiling tiles in storage area behind reach in coolers to be missing and damaged. Instructed person in charge to replace and maintain.
56. Adequate ventilation & lighting; designated areas used
- comments: observed build up of dust and grease on filters above hot cooking equipment. Instructed person in charge to clean and maintain.
