Instructed to provide at least one food employee with city of chicago certified food manager training and maintain original certificate(s) on-site and displayed in public view at all times of food operations. - comments: observed hand soap dispenser provided at the warewashing area 3-compartment sink. Instructed to remove hand soap dispenser and provide hand soap and other handwashing aids and devices only at sinks designated for handwashing and maintain.

- comments: observed several tcs food items at inadequate hot-holding temperatures: cooked rice at 116. 1f, cooked peas at 110. 7f. Instructed to denature and discard all tcs food items at improper hot-holding temperatures and maintain all tcs food items at 135. 0f or above for hot-holding. Approximately 20 lbs of food valued at approximately $80. 00 discarded on-site. Person-in-charge denatured and discarded all listed tcs food items on-site at time of inspection. Prioriy violation. 7-38-005 citation issued.

- comments: observed upright cooler in the main kitchen area missing internal ambient air thermometer. Instructed to provide at least one internal ambient air thermometer in a conspicuous and easily-viewable location in all equipment used to cold-hold tcs food items and maintain at all times.

38. Insects, rodents, & animals not present
- comments: observed no pest control log book provided on-site at time of inspection. Instructed to provide pest control log book from contracted pest control service with all documentation required by the law and maintain available for inspector review on-site at all times. Priority foundation violation. 7-38-020(c) citation issued.
