14. Required records available: shellstock tags, parasite destruction
- comments: observed raw frozen salmon in kitchen freezer with no records of parasite destruction on site. Person in charge stated that salmon is frozen on site. Instructed to maintain records of temperature of salmon before freezing, and time and temperature once frozen. It is required that item is stored in food safe containers at a temperature of -4 f or below for a minimum of 7 days. Alternatively, facility may purchase from supplier and maintain records of parasite destruction on site from supplier for 90 days. Approximately 10 lbs valued at $140 discarded and denatured. Priority foundation violation 7-38-005. Citation issued

25. Consumer advisory provided for raw/undercooked food
- comments: noted no consumer advisory for raw sushi products offered on menu. Must provide a disclosure and reminder statement on menu board regarding raw sushi. Instructed to maintain. (refer to cdph consumer advisory handout) priority foundation violation #7-38-005. Citation issued

39. Contamination prevented during food preparation, storage & display
- comments: noted no door with lock between rear dry and frozen food storage area and shared mop sink. Business shares mop sink with neighboring business. Instructed not to share space or install door with lock to keep food storage area protected to prevent potential tampering of stored food items.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: noted microwave in kitchen above south prep table in need of cleaning. Instructed to clean and maintain.

55. Physical facilities installed, maintained & clean
- comments: noted floors underneath fryers in kitchen and walls/ ceiling above to-go containers in kitchen with grease deposits and splatter. Instructed to clean and sanitize all surfaces and maintain.

57. All food employees have food handler training
- comments: observed no proof of training for all food handlers. Instructed to provide and maintain records at all times.

60. Previous core violation corrected
- comments: noted previous core violation #58 not corrected from report #2587933 conducted on 1/11/2024: no food allergen training certificates on premises. Instructed all city of chicago certified food managers must have food allergen training certificates. Must correct core violations by the date on the report. Priority foundation violation #7-42-090. Citation issued.
