38. Insects, rodents, & animals not present
- comments: observed approx. 40 live cockroaches (adult and juvenile) inside and around the stove and deep fryer and on the walls and floor of the rear food prep area. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed several dead cockroaches on the floor next to the deep fryer and under the dish machine. Management instructed to clean and remove all dead insects.

49. Non-food/food contact surfaces clean
- comments: clean the grease and food debris buildup around the door of the oven, inside of the deep fryer, on the legs, sides and underside of the wok station, the gas lines behind the cooking equipment, exterior of the grease trap, and the top of the dish machine.

55. Physical facilities installed, maintained & clean
- comments: clean the grease on the wall behind the cooking equipment.

56. Adequate ventilation & lighting; designated areas used
- comments: clean the grease buildup on the ventilation hood and filters above the cooking equipment.

58. Allergen training as required
- comments: observed 2 certified food managers without a valid allergen training certificate. Management instructed that all certified food managers must show proof of allergen training.
