- comments: observed a gap along the bottom of front, rear, and side doors must make doors tight fitting to prevent insect entry.

- comments: observed an infestation of appx. 1,000 or more mice droppings scattered on floor along wall bases of all cooking equipment, reach-in coolers, hand sinks, 3- compartment sinks, furnance room, toilet room, rear storage room, tray storage area, pop storage area, inside of fryer cabinets, on lower shelving units of all prep tables and storage cabinets. . Also evidence of a live gray small mouse run behind glass reach in cooler and then again while sitting at prep table typing report. Also evidence of appx. 20 or more live small flies flying around and on containers of food on the rear prep table. Must remove droppings and clean and sanitize areas. Must have a pest-control operator service premises on or after 10/23/24. (priority 7-38-020(a)(citation issued)

- comments: must repair or replace worn door gaskets on 3-door prep cooler, 1-door glass reach-in cooler, and french fries reach-in freezer.
