- comments: 6-501. 111 observed evidence of insect activity on site. Observed about 10 live small flies flying around and on the boxes and ceiling of the food prep area. Instructed to eliminate live insect activity, clean and sanitize affected areas, and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).

- comments: 6-202. 15 observed a 1/2 inch gap between the side entrance doors that lead directly outside. Instructed to make said doors tight fitting and rodent proofed.

- comments: 2-402. 11 observed a food handler prepping drinks without an adequate hair restraint. Instructed to provide an adequate hair restraint, such as a cap or hair net, for all food and drink handlers at all times.
