- comments: observed no hand washing signs or posters near hand washing sink in front and rear prep areas. Must provide.

- comments: observed ice scoops stored inside ice machine located in front prep area. Must properly store ice scoops in a clean container to prevent contamination.

51. Plumbing installed; proper backflow devices
- comments: observed leaking faucet base at the rear 3-compartment sink. Must repair and maintain. Additional water pressure needed at the exposed hand washing sink located in front prep area. Must repair and maintain.

- comments: observed heavy dust build up on ventilation vents inside employees washroom. Must clean and maintain.

Prep area wiping cloths must be held in a clean container with a sanitizing solution. #45 all stored items throughout the rear prep area must be elevated from the floors. #47. Noted a wooden block being used in between cooks line coolers as a knife holder. Wood cut with deep grooves and encrusted food debris. Must provide a washable knife holder and remove wood. #49. Exterior of both cooks line coolers noted dirty with food splatter. Must clean and maintain. #57. No proof of food handler training or certificates for all employees. Must provide. Priority foundation violation 7-42-090. Citation issued.
