- comments: : 6-201. 11--- all expanding spray foam being used as a sealant throughout the facility must be cut/smoothed to be cleanable. ----floor at the entrance to the first floor rear prep mixing room in poor repair with damaged, uneven tiles. Must repair to be smooth and cleanable. ----noted water damaged ceiling tiles in several areas throughout the facility. Must replace. ----missing tile grout at the basement three compartment sink. ---missing damaged wall tiles at the basement donut prep station. Must replace/repair all.

- comments: 2-102. 13--- no proof of food handler training or certificates for all employees. Must provide.
59. Previous priority foundation violation corrected
- comments: 8-404. 13 (b:3) priority foundation violation from report #2567591 on 11-28-22 was not corrected. #60- #51- 5-205. 15-observed hand washing sinks on not maintained. The hand washing sink only works by turning on the knob below the hand washing sink. Instructed manager that hot and cold water knobs must be repaired for water to run properly. #55- 6-501. 11- observed damaged wall tiles in the mixing prep room on the first floor. Instructed to replace new wall tiles that are smooth and easily cleanable. Priority violation 7-42-090. Citation issued.
