38. Insects, rodents, & animals not present
- comments: observed 35 mice droppings scatterd on the floor in front of the deli meat counter. Must remove and detail clean affected area. Also must call pest control for service. Priority foundation citation issued #7-38-020(a).

49. Non-food/food contact surfaces clean
- comments: observed cardboard used to cover shelving inside the walk in cooler. Must remove and maintain smooth and easily cleanable. Must clean the interior of all cooler and defrost deep freezer in the rear.

51. Plumbing installed; proper backflow devices
- comments: observed the faucet neck at the 3 comp sink in the front prep area with a leak. Must repair and maintain.

55. Physical facilities installed, maintained & clean
- comments: floors need detail cleaning along the wall base and incorners through to remove spills and/or debris.

57. All food employees have food handler training
- comments: observed food handler training certificate expired. Instructed manager to comply with the idph food handler requirments or ciation will follow.
