- comments: missing handwash signage at front handwash sink. Instd to obtain and maintain

- comments: dust accumumulation on vent in walk in cooler. Instd to clean and maintain same.
55. Physical facilities installed, maintained & clean
- comments: cardboard lining on floors through out. Instd to remove cardboard so as floor to be a smooth, even and easily cleanable surface.
60. Previous core violation corrected
- comments: previous core violations not corrected from 8/25/22 report 2562758 not corrected: 37 must label food storage containers when food is not in original package. 38 observed an appx. 1/2-3/4' gap along bottom of rear door. Must make door tight fitting. 39 observed several large buckets of ice cream stored on floor inside of walk in freezer. Must elevate to prevent cross-contamination. 40 observed personal belongs stored throughout food and paper goods storage shelves. Must store seperately from items and maintain 41 must store wiping cloths in a sanitizing solution when not in use on cutting boards. 47 must repair or replace worn door gasket on walk in freezer,damaged trim on interior panel of ice cream chest freezer on service line. Must eliminate crates used for food storage throughout rear storage area. 49 must clean debris build up from walk in cooler shelving units. 51 must repair or replace leaky pipe at rear hand sink and leaky pipe underneath 3- compartment sink. 55 must scrape and paint chipping paint on wall near lockers. 55 must repair or replace stained ceiling tiles in dining area,damaged floor tiles in prep area,loose and damaged wall bases in toilet room. 56 must repair or replace burntout lights throughout premises.
