- comments: observed no valid original city of chicago sanitation certificate on site at the time of inspection. Instructed person in charge that valid original certificate, as well as certified manager, must be on site at all times. Priority foundation #7-38-012. Citation issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed mold-like black slime on the interior of ice machine dripping on ice stored in bin (ice used for human consumption). Instructed person in charge to empty ice, sanitize machine and maintain. Priority foundation #7-38-005. Citation issued

- comments: observed large bin with sugar not labeled with common name. Instructed person in charge to label all non-original food storage containers with common name and maintain.

- comments: observed food debris and trash on the floor in the rear dry storage area under the wire storage rack. Instructed person in charge to clean and maintain.
