- comments: observed food handler in the kitchen handling food with no hair restraint. Instructed to provide and maintain at all times while in food prep areas and handling food.

41. Wiping cloths: properly used & stored
- comments: observed wet wiping cloth laying on the cutting boards and prep table in the kitchen. Must properly store wiping cloths in a sanitizing solution between use.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed torn door gasket inside of the 2 door prep cooler. Instructed to replace.

- comments: observed dough stored in plastic grocery bags in the walk-in cooler located in the basement. Management instructed to use proper food grade storage bags and containers only.

- comments: observed the interior of the microwave in the kitchen soiled with food derbis and splash. Instructed to clean.

- comments: observed food splash and debris in the interior of the walk-in cooler located in the basement. Instructed to detail clean.

- comments: observed a leak at the handles and facuet of the three compartment sink. Instructed to repair and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed food debris and spills on the floors and surfaces throughout the facility. Instructed to detail clean under all shelving units, interior of prep dish area, around dish machine, prep tables and all cooking equipment to remove spills, food debris, dirt buildup and/or grease. Instructed to increase cleaning frequency and maintain at all times.

56. Adequate ventilation & lighting; designated areas used
- comments: observed grease and dust accumulation on the ventilation hood and filters above the cooking equipment in need of cleaning. Instructed to detail clean and maintain at all times.

60. Previous core violation corrected
- comments: observed previous core violation #52(no grease trap at 3-compartment sink. Must provide) not corrected within 90 day time frame on the inspection report #2345615 dated for 11/14/19. Instructed person in charge that the violation shall now classified as a priority foundation item due to noncompliance.
