- comments: noted hand wash sink at the bar area inaccessible with buckets and dishes stored in the basin of the sink. Manager. Instructed to have all hand-wash sinks accessible at all times. Violation also noted on report dated 01/08/2024. Priority foundation violation #7-38-030(c). Citation issued

- comments: observed no soap and paper towel at front prep handwashing sink. Informed person in charge that soap and paper towels must be available at hand sinks at all times. Food handler obtained paper towel for hand sink during time of inspection priority foundation violation #7-38-030(c). Citation issued.

14. Required records available: shellstock tags, parasite destruction
- comments: observed no shellstock tag attached to container of mussels in upright cooler in main prep area at time of inspection. Informed manager that shellfish shellstock tag must remain attached to container until container is empty. Instructed to provide approval sources of shellfish and maintain tags for 90 days on premises. Manager voluntarily discarded 10 lbs, at a value of $20. Priority foundation violation # 7-38-005. Citation issued.

- comments: observed approximately 6 live roaches located in the following areas in the main prep area: in the corner near the cooking equipment, on the wall above the three-compartment sink, on the wall near the cold-hold unit, and the drying rack of the dishwasher. Instructed person in charge to have pest control service perform pest control services on premises to minimize or eliminate pest activity. Priority foundation violation. #7-38-020(a). Citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: food containers stored on floor inside the walk-in-freezer and attic. Must provide proper, raised shelving to store food containers 6" off floor.

- comments: noted serving plates on shelves at the kitchen prep area not properly stored. Instructed to invert all serving plates at the dining tables to prevent dust accumulation.
